Drifting eyes on eccentric gaze
A 55-year-old is examined for unsteadiness. On sustained lateral and upward gaze, the eyes drift slowly back toward the centre, with corrective jerks in the direction of gaze. Vertigo is absent.
Findings
- Nystagmus that beats in the direction of eccentric gaze
- Slow centripetal drift between corrective jerks
- Bidirectional — present on both left and right gaze
- Normal head impulse test
