Oscillopsia and imbalance in the dark

A 64-year-old reports that the world appears to bounce when walking and that balance is markedly worse in the dark or on uneven ground. There are no discrete vertigo attacks. The patient previously received a course of an ototoxic medication.

Findings

  • Oscillopsia during walking
  • Bilaterally abnormal head impulse test
  • Markedly worse balance with eyes closed
  • No spontaneous nystagmus, no vertigo attacks
