A 34-year-old with a long history of migraine with aura reports recurrent episodes of spontaneous vertigo lasting from one to several hours, sometimes with photophobia, sometimes with head-motion intolerance. Hearing is subjectively normal; serial audiograms over a year are stable. There is no aural fullness.
