Loud sounds make the room spin

A 42-year-old architect describes a year of episodic vertigo triggered by loud sounds, particularly the alarm bell of her bicycle. She has noticed she can hear her own heartbeat in her right ear and reports that her voice sounds 'too loud inside her head'. Tympanometry and pure-tone audiometry are unremarkable.
